What to report
Use this route for a mark, statement or document that may misrepresent an IESQ relationship or result. Examples include a claim outside an issued scope, use after a relevant status change or a mark suggesting an assessment that the record does not support.
Evidence to include
Identify the exact claim, location and date observed. Include the record identifier where available and explain the concern. A direct page link or clear image can help establish the presentation without requiring an extensive unrelated technical file.
Reporting route
The report form separates the claim, its location and the reason for concern. State whether you hold relevant first-hand information or are reporting an apparent inconsistency. A report is a request to consider the matter, not a public finding against another party.
Information handling
Information is used to examine the report and may need to be checked with the parties concerned. Confidentiality and disclosure depend on the matter and applicable obligations. Do not include information that you are not entitled to share.
Consideration and feedback
The matter is considered against the relevant records and use conditions. Additional information may be requested. The ability to disclose an outcome can depend on confidentiality, the reporter's role and any applicable investigation or legal requirements.
Submit the report
Provide enough information to locate and understand the issue, then retain the reference. Check the identifier and issuing body before submitting. A verification no-match alone does not establish misuse.
Other concerns
Use complaints for dissatisfaction with service or conduct, and appeals for an eligible decision challenge. Those procedures have distinct purposes and conditions.
